Your privacy, and the right place to share.

This notice explains information collected through this public website. It is separate from any privacy notice or consent documents that apply to an actual care relationship.

Information collected here

General inquiries include your name, phone number, optional email address, preferred contact method, and any patient-free message you choose to provide. Partner setup requests include organization and professional contact information, service interest, and general timing. Submission is voluntary; you can call instead.

How website information is used

Contact information is used to respond to an inquiry and coordinate the requested conversation. Netlify provides website hosting and general contact-form processing; it does not process the dedicated assessment form’s answers. Its infrastructure may process IP addresses, browser and device information, request times, and security logs to deliver and protect the service. Only share information needed for the general inquiry.

Health information and HIPAA

General contact, SMS, partner-setup, and career forms must not be used for protected health information (PHI). The dedicated assessment form described below uses a separate Google Workspace pathway. Do not enter patient names, dates of birth, diagnoses, insurance details, records, or information about someone else’s health. No uploads are accepted. Call the care team to arrange an appropriate channel for care information.

The Health Insurance Portability and Accountability Act (HIPAA) applies to covered entities and business associates in defined circumstances. Hosting this website does not establish HIPAA compliance. This website privacy policy is not a HIPAA Notice of Privacy Practices. Read our Notice of Privacy Practices for health-information rights, permitted disclosures, and privacy requests. Call the office to confirm an appropriate channel before sharing health information.

Health privacy rights

Where HIPAA applies, rights can include access to and correction of records, confidential communications, requesting certain restrictions, an accounting of certain disclosures, and a copy of the applicable privacy notice. Contact the office to ask how to exercise applicable rights. You may also raise a health-privacy concern with the U.S. Department of Health and Human Services Office for Civil Rights. You do not need to use a public website form to make that request.

Cookies and local browser storage

This site does not install advertising trackers or analytics scripts. A session-storage preference remembers whether you have dismissed the pathway chooser during your browser session. Hosting and security services may use information necessary to operate the website.

Security, retention, and your choices

The site uses HTTPS and limits public form fields, but no internet transmission is risk-free. Call to request access to, correction of, or deletion of your general website inquiry, subject to applicable recordkeeping obligations. Ask the office about retention for your specific request. Avoid sensitive information in any follow-up that has not been identified as an appropriate secure channel.

Children and other websites

The website is intended for adults seeking care or professional information, not for children to submit information independently. External links have their own privacy practices. Material changes to this notice will be reflected on this page with an updated effective date.

Dedicated in-home assessment form

The assessment page loads a form served by Google Apps Script in Our Hearts’ Google Workspace. It collects your contact details and relationship to the person seeking care, care area, payment and service interests, and optional age, support needs, requested services, service address, pets, schedule, and referral-placement context. Only share your own information or information you are authorized to discuss. Do not upload records or enter Social Security numbers, insurance identifiers, diagnoses, or treatment instructions.

Answers are sent directly to Google Apps Script and saved to a restricted Google Sheet for Our Hearts’ authorized intake review, not to ordinary Netlify Forms. Google’s HIPAA Business Associate Amendment has been accepted for this Workspace; that agreement does not itself certify every organizational practice. We use the information to assess the request and arrange follow-up. Optional details may instead be discussed by phone. The website does not place answers in URLs, advertising tools, email notifications, or local browser storage. Contact our office about access, correction, or retention, subject to applicable recordkeeping duties.

Appointment scheduling with Google Calendar

The assessment and interview pathways share a Google Calendar booking page. After the assessment request is saved and confirmed, the page opens the appointment calendar. Scheduling details you provide there are handled by Google and the calendar organizer under their applicable privacy practices. Our website does not copy inquiry answers into the calendar, read your calendar, or store a booking confirmation. Use scheduling contact details only; do not enter service addresses, health information, patient records, or urgent-placement explanations. Confirm the appointment type and location with our team.

Caregiver and referral contact forms

The caregiver form collects professional contact details, role interest, experience, training category, general work area, and work availability to discuss opportunities. Do not include credential numbers, Social Security numbers, medical information, client information or documents. The referral contact form collects organization details, the referring professional’s contact details, the reason to connect and a preferred callback window. It does not collect or transmit patient referrals or records. These general contact requests are processed through Netlify Forms for authorized administrator review; no patient-information system or Google Sheets connection is represented. Text messaging is optional and requires the separate unchecked consent box.

Text messaging and mobile information

Our Hearts Home Care collects your name, mobile number, and optional SMS consent through the appointment form and general contact form. We use this information to respond to your request and, only when you opt in, send SMS customer care and product or service delivery notifications. We retain the submitted consent choice, disclosure version, and submission record to document your permission.

Your mobile information will not be sold or shared with third parties for promotional or marketing purposes. Text messaging opt-in data and consent are not shared with third parties for their own purposes. Service providers that process our forms or deliver messages may process this information only on our behalf to provide those services, subject to appropriate confidentiality and data-use restrictions. We may also disclose information when required by law.
