What the forms collect

The family form collects general contact details. The partner setup form collects organization and professional contact details. Both are processed through Netlify Forms so an authorized site administrator can review the submission.

What must not be submitted

Do not submit patient names, dates of birth, diagnoses, insurance information, medical records, identifiers, attachments, or other sensitive health information. This website is not a patient portal.

Tracking

This build does not use analytics, advertising trackers, or nonessential cookies.
